To Create a Change Request from a Template
You can create a change request from a change request template. This lets you quickly create a change request with predefined attributes, relationships, and security.

Use either the Templates or Classes tab to select the template you want.
On the Templates tab, templates are presented in a straight list that you can filter. On the Classes tab, templates are found under the class group they belong to.

OK.
A change request is created based on the selected template.
